TSB releases investigation results following tug boat incident

The Transportation Safety Board of Canada has completed its investigation into the sinking of a tug boat in the Northumberland Channel near Nanaimo.
The incident happened back in May of 2016 while the C.T. Titan was passing another tug boat, the Albern.
The TSB investigation found that the master of the Titan left the navigation controls unattended for between 6 and 8 seconds but it was enough to veer off course.
The incident resulted in the Titan slamming into the Albern causing it to capsize and sink.
The investigation found risks related to unsafe work practices, the lack of company safety management processes, and insufficient regulatory inspections.
Since the collision, in which no one was hurt, the company, which owned both vessels, has taken steps to prevent such incidents in the future.
